Ongoing Case Management
Case management provides continuity of care and coordinated support across systems. Our case managers collaborate with residents to assess needs, set attainable goals, and develop individualized settlement plans. Plans often include connections to language training, trauma‑informed counselling, immigration and legal supports, education, employment programs, and community‑based services.
Case management includes:
• Comprehensive intake and needs assessment
• Individualized settlement planning with goal‑setting and progress tracking
• Advocacy and navigation support across healthcare, legal, education, and social services
• Regular wellness check‑ins and transition planning during key milestones
• Coordination with community partners to ensure timely access to services
